As you can't do anything for the ill at the moment, leave them and explore the Farmlands - visit Farmer Hugh. His harvest was poisoned and he asks you to investigate. West of the poor quarters is a cave, enter it and you'll find Orc Tipsix and a lot of poison barrels. As he won't tell you what's going on and attack you - kill him and take the golden key he dropped. Report this to Hugh and show him the key - the next quest: Where does the key fit? If you've been in the cave before you have been to Hugh, Tipsix won't be there, he is triggered after your visit.
